Job Title: Mental Health Skill-building Case Manager

Supervisor: Clinical Director/Licensed Mental Health Professional

Description: Provide mental health skill-building services to individuals 18 years and older. Responsible for development and implementation of individualized services plans in accordance with program description.

Responsibilities:
  • Provide direct services to clients, including linkage, advocating, supporting, and providing education as needed.
  • Prepare written individual service plan, incorporating input from clients, involved service providers, and clearly identifying problem areas and needs, strategies, and service objectives.
  • Respond to crisis situations within twenty four hours.
  • Link clients with external programs or services, such as health services, recreational activities, childcare services, financial resources, employment resources, AA/NA groups, transportation resources, and others.
  • Collaborate and coordinate with other service providers and professionals, such as attorneys, social workers, and probation/parole officers.
  • Complete all progress notes and quarterly reports in a timely fashion.
  • Recommend appropriate services for clients.
  • Assure clients are informed of his/her human rights.
  • Maintain weekly contact with referring agency.
  • Conduct discharge planning in accordance with plan description.
  • Attend all meetings as required.
  • Provide accurate time sheets.
  • Advise Clinical Director of the state of clinical services, its staff, and its needs and solicits their assistance with implementing solutions to problems.

Strong writing and case conceptualization skills and reference to a methodology. The successful completion of a criminal history or background investigation is required. JOB DESCRIPTION

Job Title: Clinician

Supervisor: Director of Services

Description: Position is responsible for the coordination and implementation of all Substance Abuse Clinical services offered to adult clients. This position is responsible for staff training and supervision of clinical issues as well as overall counselor supervision in the program and group setting. This position is responsible for overall clinical case management of clients, to include medical, counseling, educational, vocational, and recreational services. This position will require on call services as needed. Duties include but are not limited to diagnostic initial assessment, prior authorizations, intakes, re-assessments, implementing treatment plans, conducting individual and family counseling, reviewing quarterly reports, discharge summaries and re-evaluating clients’ needs for continued stay. In addition, clinician responsible for the overall S.A. case management of clients in the group settings and will maintain a client caseload of 1 to 10 clients. This position is responsible for facilitating, (psycho educational, Substance Abuse, Anger Management, Life Skills, Family and Parenting) Groups. This position will act as a liaison between clients, referring agencies and other communities based organizations providing crisis intervention as needed. Advocating for the clients, whenever necessary experience with family systems, juvenile courts, crisis intervention, and parenting skills helpful. A cell phone, home telephone and a DMV license is required.

Job Title: Director of Services Crisis Stabilization

Supervisor: Clinical Director

Description: To provide leadership and management of all case managers to monitor services delivery to clients. To monitor the overall day-to-day operations of the program.

Duties & Responsibilities:
  • Provide oversight of the scheduling, billing and invoicing process.
  • Review all billing for accuracy and monitoring all appropriate paperwork (i.e. progress notes, quarterly, discharge reports, etc.)
  • Provide oversight of the productivity of staff to include the management of assigned cases and follow through of expected hours for each staff member.
  • Provide direct supervision of all staff on a monthly basis to monitor their cases.
  • Provide oversight of the accountability of paperwork receivable; weekly review of the billing sheets, follow up on any outstanding paperwork.
  • Provide oversight of quality assurance of all client files to ensure compliance.

Responsibilities will include providing oversight/supervision for: assessments, individual service plans and discharge planning, clinical supervision, the coordination and implementation of clinical services and clinical case management offered to clients. A thorough understanding of DMAS policies and procedures and DBHDS policies and procedures is required.

Duties & Responsibilities:
  • Prepares written diagnostic assessments to be conducted face to face, assessing clients form multiple perspectives, including, psychological/emotional, social, academic, medical, behavioral, substance abuse, skills, strengths, barriers to economic self-sufficiency, treatment history, and others, and utilizing various assessment instruments.
  • Generate a Magellan for each assessment completed.
  • Assist mental health worker in creating an initial ISP within 24 hours and an ISP within 30 days of treatment. Sign off on each initial ISP and ISP personally in a timely manner.
  • Provide supervision to (QMHPs and QPPs) providing case management services. Meets monthly with QMHPs and QPPs to provide clinical guidance, assess dynamics and needs of each case, determine the appropriateness of strategies, and assess progress of service delivery and planning.
  • Attend monthly staff meetings and provide clinical guidance to QMHP or QPP and staff of Be Mindful Outreach Services, LLC.
  • Perform clinical file reviews to ensure client files are in compliance with agency, DMAS, DBHDS and state code policies and procedures. Document clinical file review and make recommendations for changing as appropriate. Notify the Executive Director of findings in a timely manner.
  • Provide consulting and additional referrals to community resources as appropriate.
